Home Modifications

Sunshine Rehabilitation Services will conduct an analysis of the existing housing accommodation to determine appropriate modifications necessary to the home, based on the needs of the client, in order to facilitate the client's reintegration into his or her family, the rest of society and the labour market. Home modifications such as ramps, grabs bars, customized counter and seating heights toilets and shower stalls may be recommended.

Hospital Discharge Planning

Sunshine Rehabilitation Services will co-ordinate the services necessary for clients requiring assistance with discharge home from hospital, following complex or multiple injuries, as a result of a motor vehicle accident. Sunshine Rehabilitation Services will co-ordinate all services with the multidisciplinary treatment team, as well as the client and the family members. The appropriate equipment and assistive devices, as well as the co-ordination of appropriate support workers may be provided based on the needs of the client to ensure a smooth and safe transition from the hospital to the home environment.
